Under the direction of the Care Management Program Manager, this position is responsible for scheduling and supporting program events and activities, organizing and communicating program data and information, and ensuring excellent customer service to program participants and stakeholders. This position supports program staff and the Program Manager to ensure referrals, events, and program operations are organized and processed in an efficient and effective manner. The Program Coordinator is expected to work independently and exercise discretion and judgment to identify problems, determine causes, and implement solutions.This role assumes responsibility for decisions, consequences and results that impact people, costs and/or quality of service within the functional area while maintaining a professional, customer service-focused presence to both internal and external customers. Receives, documents, tracks, and communicates referrals. Assigns referrals to program staff and monitors staff caseloads. Provides administrative support to programs including scheduling, taking meeting minutes, and maintaining documents and inventory of program materials.Manages and tracks staff training, orientation, and program expenditures. Provides prompt and clear electronic, written and verbal communication with program staff. Manages and assists with strategic administrative projects and events related to the expanding needs of the department. 40 hours per week Day Shift 8:00 AM 5:00 PM Work 1.Associates degree required. 2.
