The RN Case Manager is responsible for direct patient care, coordination of overall home care services, and documentation of care for agency clients in accordance with quality standards and under the guidance of a Clinical Supervisor.Performance Responsibilities and StandardsEnsure quality and safe delivery of home health services including case managing of client caseloads, developing a plan of care based on client assessment and OASIS evaluation, using an interdisciplinary approach, and ensuring documentation meets quality standards.Complete all OASIS evaluations; ensure documentation reflects nursing care and interactions, is clear and concise, and is completed timely.Coordinate with LPNs; provide guidance and education involving patient care, work with supervisors and PCC to manage patients using a team approach, and utilize LPNs appropriately.Supervise/oversight of paraprofessional staff related to patient care; supervise home health aides, communicate relevant information, and ensure care plans are followed.Comply with all policies, procedures, and quality initiatives; maintain confidentiality; adhere to infection control, regulatory requirements, and organizational standards.Maintain and update clinical knowledge regularly.Job QualificationsCurrent NY State RN license; BSN preferred.Valid drivers license, auto insurance, and vehicle; Fleet Program available.Self-organized, able to work independently in a fast-paced environment.Experience with pediatric or MCH care is a plus.Proficient in computer skills, including electronic medical records.Excellent communication skills.Physical RequirementsAbility to sit or stand for most of the day.Physical tasks such as lifting, patient transfers, bending, standing, walking, and stair climbing.Ability to drive in all weather conditions.Sufficient vision/hearing for communication.Use of telephone and computers.Bloodborne Pathogens Exposure Category III.Compensation & BenefitsSalary: $72,415.71 - $83,922.76 annually, with a $15,000 sign-on bonus (restrictions may apply).Retention BonusesMentoring Program401K with employer matchOn-site gymWork/life balance with no rotating shiftsMedical, Dental, VisionTuition Reimbursement (BSN after 6 months)Partially funded HSARecognition PlatformPaid Time Off, Holidays, Sick LeaveDisability benefitsEmployee Assistance ProgramAbout Nascentia HealthLeading in home, post-acute, and community health care, focused on holistic, patient-centered care in home environments, aiming to improve outcomes and reduce unnecessary hospital visits.
