Company: Mdecins sans Frontires

Mdecins Sans Frontires (MSF), an international medical humanitarian association founded in 1971, provides medical assistance to populations whose lives are threatened, mainly in cases of armed conflict, epidemics, pandemics, natural disasters, or exclusion from healthcare. The French section is present in around thirty countries.

Over recent years, cervical cancer has become a significant health issue in Malawi, accounting for 40% of all cancers among women and causing an estimated 2,300 deaths annually. MSF has established an operating theatre, an 18-bed inpatient ward, a day clinic, and palliative care services. Additionally, MSF supports the Ministry of Health in providing chemotherapy for cervical cancer patients and is exploring possibilities for implementing radiotherapy. These efforts aim to make a lasting difference in the lives of affected women.

Brief description of Malawi

Malawi is a landlocked country in Southeastern Africa, covering over 118,484 km2 with an estimated population of approximately 20 million. The population is predominantly young, with 44% under 15 years, 51% between 15-59 years, and 5% aged 60 or older. Malawi has been relatively peaceful since independence in 1964, with stable governments and democratic elections since 1993. Despite economic reforms, it remains one of the poorest countries, heavily reliant on agriculture, which employs nearly 80% of the population and is vulnerable to climatic shocks.

Description of MSF Assignment in Malawi

Since 2017, MSF France has been developing a comprehensive cervical cancer program aimed at addressing inequalities in screening, early diagnosis, and treatment in low- and middle-income countries. The program targets all stages of the disease, from HPV infection and precancerous lesions to invasive and metastatic cancer, including end-of-life care. In collaboration with the national Ministry of Health, MSF provides an integrated approach to diagnosis and management of women''s cancers, involving vaccination, screening, diagnosis, treatment, surgery, chemotherapy, and palliative care, delivered by a team of expatriate and national specialists.

Testimonial from the national gyneoncological surgeon

"The unavailability or inadequacy of treatment options for women diagnosed with cervical cancer in Malawi has resulted in pools of women at risk of dying from undiagnosed, diagnosed but untreated, or inappropriately treated cancer. I joined MSF in 2019 to help address this issue. It has been an enriching learning experience, and we have impacted many women''s lives. Over nine ESGO oncologists have rotated through the project, imparting valuable skills. Our holistic approach ensures that even the most disadvantaged women are reached. Despite challenges, we keep moving forward. I am grateful for the friendships formed." - Dr. George Chilinda, Malawi

Testimonial from an expatriate gyneoncological surgeon

"My first mission to Malawi opened my eyes to the severity of the cervical cancer problem. Treating many late-stage patients is challenging, but I am impressed by the high standards of care MSF provides, even in a low-income setting. Our team, comprising skilled surgeons, gynecologists, nurses, psychologists, and logisticians, ensures quality treatment. The professional management made my start smooth. The friendships Ive formed here will make my departure difficult." - Dr. Sebastian Hentsch, Germany
