**POSITION SUMMARY**:The role of the Residential Counselor is to provide individuals with assistance in all areas of daily living, community integration, education, counseling and management of finances. The counselors provide emotional support and on-site integrated, goal-oriented, individualized services to assist program residents with developing and optimizing their strengths, skills, and available resources. The counselors demonstrate respect and empathy for the individual and consideration for his/her dignity and self esteem while providing teaching and skill building focused on developing independence while optimizing their quality of life.**RESPONSIBILITIES**:Assist in the daily operation of the Program.Utilize all required assessment tools to assess clients functionality and mental health status, needs and preferences, and ongoing appropriateness of services.Assist clients in developing and implementing individual recovery plans. Coordinate recovery service planning to include members of clients treatment team and clients natural supports. Conduct regular reviews of recovery plans and modify as needed.Ensure appropriate referrals and linkages are made to other parts of the mental health system and other community resources, including clinical, vocational, psychosocial, educational and natural supports. Provide advocacy as appropriate.Demonstrate the ability to understand the delivery service required for TCM (Targeted Case Management), and the IDDT (Integrated Dual Diagnosis Treatment) principles utilizing Motivational Interviewing.Demonstrate an ability to develop a basic level of competency for providing services to clients with co-occurring disorders.Assist all Program clients in the areas of health and safety and in the acquisition of independent living skills, including personal hygiene, meal planning, cooking, cleaning and money management.Provide rehab interventions as directed by resident recovery plan, including facilitating rehab groups.Provide transportation. *Not permitted to drive 10 or 12-passenger vans.Assist clients in the scheduling of personal appointments. May accompany clients to appointments.Read and write in communication logs, progress notes, and other departmental communication tools to ensure understanding and adherence to changes in clients recovery plans and/or client/programs needs or concerns.Provide crisis assessment and intervention, utilizing clinical and emergency back up as indicated. Communicate client needs with hospital emergency rooms and emergency personnel.Maintain and update case records, on paper or electronically, with required progress notes documenting progress toward goals and objectives.Enter daily service activities into the data base and insure the data accuracy.Adhere to Agency policies and procedures, including ongoing compliance with accreditation standards.Attends training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.May be required to use own vehicle on company business.Regular and consistent attendance is an essential function of this positionMay perform other assignments and duties as assigned QUALIFICATIONS:Two years of college education or High School Diploma or Equivalent and 2 4 years direct care work experience.Must have and maintain a valid CT drivers license.
